Ceiling is double 5/8 drywall with GG and clips/channel. Between joists above the room is 5" HVAC that is only opening up to room above. Should I replace 5" HVAC with flex duct even though it does not open into the room? I also avoided sketching a central vac pipe and an additional HVAC run that also runs in this area to the room above because I think I will be able to relocate those....unless the effort is not worth the benefit?

I'd say there's no point replacing the ducting with flex. I would recommend packing the ducts in insulation, and if possible including a foil or pvc outer mantle.

I'm planning on R19 in the joist cavities. What do you mean by a foil or pvc outer mantle?

Basically I mean't packing them in proper ductwrap insulation which usually has a silver foil bonded to the insulation on one (the outer) side. Forget the PVC mantel - this applies to pipe insulation - my error.
However, I guess the R19 will also work with the paper out - you'll have to probably wrap a wire mesh (or use a lot of duct tape) around the insulation to keep it in place - the ductwrap has a sealing strip for this.
